Stan I had about 7 covid extensions and I have entered 4 times this year visa exempt without any problems. You asked about Visas and that I can’t answer. Only visa exempt. The reason that Thailand has extended the visa exempt from 30 days to 45 is to increase tourism. So their main interest is to admit tourists. In 2019 there were nearly 40 million tourists now Thailand is not close to that number.

I had no problem whatsoever. Though I have been abroad for almost 3 months after COVID extensions before getting back into Thailand.

speak to the hospital about getting a medical visa then. The IO is unlikely to give two hoots about your treatment when you've been living here on covid extensions.

If you think about it from their perspective few tourists would stay for a year or more without some form of income - which means you could be working here illegally.

Airports generally more strict than land borders. If they’ll let you in you’d get 45 days (after October 1st) but expect issues.
