Can I submit a reentry permit application with tentative dates for leaving Thailand?

Please excuse me if this question has been asked before. For the reentry permit. On the form it asks what day you intend to leave Thailand and return. I am still waiting on final confirmation of dates but it will be a rushed trip, supposed to be soon, and I wanted to go to CW and do this in advance. If I put a date on the form but end up leaving a different day and coming back a different day is this ok?

just make up any date you want they don't care and it has nothing to do with you actually leaving it's just required on the form.

You will get a re-entry permit good for the validity of your current stamp

Awesome. Thank you very much. And just quickly, if I choose multiple, I can come and go multiple times during the validity of my stamp and not have to apply for new re-entry permit each time?

correct on a multiple re-entry permit (3800baht) you will be able to exit/re-enter and each time get stamped back in until your current stamp's expiration date.
