Do I need to issue a new re-entry permit if I change my trip dates in Thailand?

Good evening experts,

Changing dates of my trip: I have issued a single re-entry permit and mentioned specific dates of departure and arrival as requested in the form. If I change my trip's dates, do I need to issue a new re-entry permit, or still can use the same one?

the dates on the form are just that; b/s dates you put down to "fill in the blanks" on the form. They have nothing at all to do with when you actually travel.
