Re-Entry. What happens when you choose to travel on a date that's different from the one in the Re-Entry form ?Do you need to get a new stamp? Thanks in advance.

Do you mean re-entry permit? If so you can enter any day up to and including the date on the stamp.

The date you put on the application form is just any date .. you may leave on any valid date of your choice.
There will be a date for which the re-entry permit is valid. To match the date you are permitted to stay in Thailand with your current Visa or Extension of Stay stamped in your passport.
