Are exit (especially) and re-rentry dates given when applying for re-entry permits on O visa strict?
That is, if something went wrong with travel plans, would you be expected to apply for new permit?

No those dates aren’t set in stone. If you say on the form you’re leaving on abc date but actually leave on xyz date it’s not an issue. Exit or re entry dates you put on the form serve no purpose really. Why you’re required to write them is anyone’s guess.
