TLDR : Answer Summary
You cannot obtain your COVID extension stamp from Chiang Mai immigration if your house is registered in the TM30 in Surat Thani. You must apply for your extension at the immigration office where your residence is registered.

No you can not; you need to get your extension of stay where you're registered at. Don't forget, you may get an "under consideration" stamp, and will have to return to that immigration office as well to close it out.
if I can ask you, on the same subject, I got a covid emergency extension visa using the address of the condo I was renting in hua hin, if I go to phuket, is it possible to reapply 26th November for same extension at phuket immigration and if so would I need to do anything before visiting immigration on 26th ?
If you already closed out your existing 60 day extension of stay based on unable to travel due to COVID-19, and you already filed a TM30 at your new location, then you can get a new COVID-19 extension of stay at any immigration office
At most immigration offices, they issue an "under consideration" stamp for the 60 day extension of stay based on unable to travel due to COVID-19. You need to close out the under consideration stamp at the same immigration office
