Non immigrant OA visa multiple entry leave before 1 year get stamped another year second year you need re entry form correct? I am not planning on returning to US can I file for one year extension?

IF you bounce out and back in just before the VISA expires you will get stamped in for a whole new calendar year.
IF during that second year (after the visa itself has expired) you want to leave the country and return you would need a re-entry permit to keep that last entry stamp 'alive' so you would be stamped in until it expired when you came back
IF you are not leaving the country during the second year, you don't need a reentry permit.
if you go the extension route you apply for a yearly extension every year, by meeting financial requirements and turning in the correct documents. You do that year after year for as long as you want to.
there is no "permanently", you would either go back to the US and get a new O-A visa or you would apply for a yearly extension by meeting the financial requirements and submitting the appropriate documentation, copies, bank letters etc to the immigration office. You can apply for a yearly extension when you have 30 days or less left on your current stamp.
