What documents and information do I need to take and what forms do I need to fill out to get an exit and entry permit at immigration?

Hi all - I’m unable to travel back to the US at the moment because of COVID. The US Embassy just started issuing letters again for an extension so I’m planning to take this to immigration to request a 30 day extension past the Sept. 26 amnesty date. I’m wondering if anyone else here has done the same and if so, do you know what documents are necessary to present to immigration? Thanks!

Must thank Tod for your advice yesterday. I went to immigration office at ta yang today. And yes I took all my documents rent book blue book of owner of my house. And I did have to fill in a TM 27 . So many thanks Tod
