Foreigners do not need exit permits to leave Thailand, all they need to do is hand over the passport to Immigration at land borders or on airports before departure.

For the Re-Entry Permit, which will be valid as long as your current permitted stay:

Copy ID page passport

1 passport picture

Application form you get from Immigration

1.000 Thb for Single Entry or 3.800 Thb for multiple re-entry.
