What should I do if I've lost my TM6 form while applying for a COVID extension in Chiang Mai?

Chiang Mai Immigration - Lost T.M.6 form - applying for a covid extension.

I am applying next week for a 60 days covid extension on a tourist visa at Chiang Mai immigration, but I cannot find my TM6 form.

1. Where you asked for the original one when you applied there?

1. I have a copy, will that be enough?

1. If it is a problem, can I get another one issued?

You'll probably be better off asking at a Chiang Mai group, but the rule of thumb is:

Yes you can. You need to go to the police and file a police report saying you lost your TM6. You will take the police report to the immigration office (which one, I do not know), and they will issue you a new TM 6.
