TLDR : Answer Summary
To extend your Non-O family visa for 30 days, you will need a TM6 form. If you have lost your TM6, you can report it at a police station to get a report and then take this to the immigration office to request a new TM6. Some users have mentioned that they were able to obtain a new TM6 at immigration without needing a police report, but this may vary.

You will need a TM6. Go to the police station and report it lost. Take the police report to the immigration office and request a new TM6.
