Please advise what, if anything, i need to do if I loose my TM.6 departure card.

Passport = America

Note: Soi 5 Immigration conveyed to fill-out a new one upon my next departure.

if you're not going to go to the immigration office before you fly out do nothing. you aren't the first foreigner to have lost one.

IF you do need to go to the immigration office, some require a police report before they will give you a new one.
