TLDR : Answer Summary
To apply for a DTV visa, a doctor's letter is required that outlines the ongoing medical treatment. The letter must be on official letterhead, detail the length of treatment, number of visits, and be endorsed by a recognized doctor at an official hospital or clinic. It should stipulate that the treatment lasts longer than six months. Applications can be submitted online, or at embassies in Laos or Vietnam, with both options being viable.

These days you really need a proper treatment plan signed off by a recognised doctor at an official hospital/clinic on that entities official letterhead. As

said make sure the treatment plan spans longer than 6 months

All applications are submitted online. If you apply via Laos you pay in person and have a brief interview once they assess your application. Both are good embassies to apply from. The letter should include length of treatment, the number of visits
