Hi there! I'm on Ko Samui right now under visa exemption stamp until 29 January.

I would like to extend my stay to 60 days by "COVID extension stamp".

1. What a correct reason should i fill in the immigration office form to get 60 days extension?

2. What additional documents may be required?

I have visited immigration office and ask them about documents which i need for extension. They see my passport, then give me: TM7, STM2, The Acknowledgement of Penalties for a Visa Overstay, Covid form. Does that mean i can apply Covid extension?

you can do it any time, they add the 30 day extension on to your current stamp's expiration, so you don't lose any days going early

There's only one reason you can get a "Covid extension stamp" and that's unable to travel due to Covid. You might struggle with this given you've just arrived but you are out nothing by asking. If you are on a visa exemption they will likely ask you to get the 30 days extension first though which will push you beyond the date you can currently apply for the Covid extension

You write your name in Russian letters.

If you are here on a Russian passport, you are not here on a visa exemption stamp.

But on a visa waiver based on a bilateral agreement.

There is no 30 days extension available for that.

A 60 days "covid-extension" would be your best bet.

You can try to ask for a 60days Covid extension. The worse thing is that the Immigration office would say no and let you apply for 30days standard tourism extension.

As it seems you are on the initial 30 days you got on entry, you have to apply first for the normal 30 day Extension of Stay based on tourism.
