This is the requirements for the Phuket immigration office. I’m assuming you mean an extension of stay from an immigration office in Thailand rather than an actual visa. Most offices will require these and possibly more. The only way you’ll get a definitive answer is to go to your local office and ask for their handout of what’s required.
If you’re actually asking about getting a visa - as in the initial sticker in your passport before you come to Thailand then the only place you’ll get that info is from your local embassy or consulate. Most have websites listing the requirements for each visa type.
