What documents are needed to apply for a DTV visa in Phnom Penh?

I applied in August and was approved. Got there at 9am in the morning on a Tuesday (very long line). Submitted the form, $400, photo, passport, passport photo page, letter from employer stating my job, salary and remote status, 6 months of bank statements and then received a receipt to return the following Tuesday in the afternoon (3-4pm). Got there, handed my receipt and received passport back with the visa. Almost no speaking with the staff. They seem uninterested in answering any questions.

I just applied a few days ago. No interview needs to be taken. Have to wait 1 week for visa stamp.
