What is the declaration form they ask for when  doing 90 day STV extension? Is it the one I filled out in U.K. or is this another one they supply? They only gave me the tm7 when I went to get forms.

I got no idea, perhaps someone who has extended a Special Tourist Visa for 90 days will weigh in

I know most offices require

proof you went thru ASQ

proof of where you will stay for 90 days

proof of insurance for another 90 days

proof of where you are staying

and copies of;

data page of your passport

the visa sticker

your entry stamp

TM6 departure card

Before you start submitting forms that you download from the internet, remember some immigration offices may want their own special forms. I know Nakhom Pathom does NOT accept the forms posted online, and they want it on their own special forms.
