What documents are required for the new 90-day reporting rules in Thailand?

Anyone can say which documents need to do 90days as per new rules. I did my last 90days in September that time it was old process only passport and file up form. Please give me a quick reply with correct information. My last date for 90days is 10 December.

I asked because different offices require different things

BUT

people are right, in Bangkok you only need a filled out TM.47, your passport and your previous 90 day report receipt
