Medical Extensions of Stay are maximum 3 months, but indeed depending on medical statement of the doctor. Some Immigration Offices want a second opinion an sent a doctor to check the condition of the patient.

You get from the doctor a medical statement and somebody else can go on their behalf with the passport and the medical statement plus 1.900 Thb fee and apply for the Extension of Stay based on medical issues.
