Have any of you extended your elite visa recently in Phuket (3 months) and did they require a negative covid test?

the cost for a yearly extension at the immigration office on an elite visa (if you can't leave/re-enter the country to get a new year stamp) is 1900baht just like it is for everyone

90 day report you mean? If you're in Phuket bring your passport to the Elite office and they'll do the 90 day report for you.

Elite visas don’t have an option for extending for 3 months. You currently don’t need a negative Covid test for any extension.
