Currently in Vietnam/ HCMC applying DTV medical.
Some notes:
-Bank statement over the last 6 months, with the final balance of 500k THB (can be transferred e.g. in the last week)
I also took a print screen of the final balance on the day I applied (sometimes the statement shows the previous date not current date, the day of applying)
-A medical certificate with condition, proof of following appointment and treatment plan for the coming 6 months. All official with paper of health center/ doctor, stamp and signature.
Showing proof of previous appointment(s) is not being asked, but I think it is a plus if you do.
At first the Embassy said "please provide appointment letter with hospital and a letter from them stating your condition for the hospital additionally, if you have any receipt/ any document proven your related project it is recommended to attach with main document."
However during the process I am being asked to show an extra document of 6 months treatment plan. Even though it was indicated in the medical certificate that I need a check-up every x months for my condition.
-I was also being asked to show ALL pages of the passport, even the blank ones. Maybe because my passport is totally new.
-Proof of location: entry stamp Vietnam, booked hotel in Vietnam, ticket proof to Vietnam/ boarding pass. I took a photo of the accommodation's invoice, entry stamp, and boarding pass.
My process so far. I thought I had done everything perfectly, so the extra "homework" surprised me a bit.
