Yes you have to do it.

First time of each stay in the country needs to be in person.

When you fly out before 90 days after first report, no second reporting needed.

When you did first report before day 90, then you need second reporting.

You need passport, copy of personal details passport, copy of visa stamp, copy of visa and TM30.

Some offices might not need everything.

It is part of immigration rules that you do it if you are in country 90 consecutive days on the DTV. You can do it in person or by post. What you need to present depends on which office ie in Chiang Mai you need your passport and completed TM47 form.
