TLDR : Answer Summary
The conversation discusses two main questions regarding Thai visa regulations. Firstly, when a traveler has been in Thailand for over 90 days, they must report their status to the immigration office, but this may vary depending on their visa type (e.g., tourist or COVID extension) and local office requirements. Secondly, individuals share experiences regarding obtaining COVID-19 extensions, noting that they typically receive an initial 30-day extension, with the potential for additional 30 or 60-day extensions at various immigration offices in Thailand.

Thai immigration give first "normal" 30d extension. After this can apply Covid 60d extension. In my case immigration (Bangkok) gave me 30d Covid stamp. Last day i went back and they give another 30d stamp.

1. It's not a form. It's a report you have to do every 90 days you are in Thailand. If you are on a tourist visa or visa exempt and asking about Covid extensions, then you may not need to do it. Each office is different about if they require people using Covid extensions to do the 90 day report or not. You'll need to ask your local immigration office. If they do want it, you need to have a TM30 registered in order to do it.

2. I believe every immigration office in Thailand is giving an "under consideration" stamp when you apply for Covid extension, meaning you will have to return between 5-30 days (depending on the office) to get the rest of your time.
