OK - with everything changing this has become confusing for me. Hopefully you guys can help me sort this.
I’m currently in Thailand on a retirement non-immigrant “O” Visa with extension of stay granted until December 2022. Up to date on 90 day reports. I have a single reentry permit -not yet used. I am fully vaxxed - Az + Pfizer in thailand, two Pfizer in USA.
I have no insurance and no Thai Pass because they weren’t required when I re-entered and quarantined in Phuket for 2 weeks in September 2021.
So now I want to do a land border crossing at Ban Laem into Cambodia, and then return to Thailand by land four or five days later. So it looks like for rentry I have to get a Thai pass and show proof of insurance until my extension of stay runs out in Dec 2022. Is this true? Or does my existing unused reentry permit change any of that? Any other important nuances I am missing?

As long as your extension is based on Non-O and not Non-OA (this visa type has its own insurance requirement) then you should be okay with only 30 days of insurance to enter the country. That will meet the Thailand Pass entry requirements.
