Hello, anyone applied to a STV (special tourist visa) recently?

The Thai Embassy in my country they still send me the same document about the requirements for apply STV.

As the document is dated on 21 Jan 2021, there still having the 14 quarantine days...

And about insurance they ask, need to be a Thai company insurance right?
