Hello, I am currently applying for DTV under soft power option in Poland. I got a letter of appointment from my dentist.

I got 3 questions :

About accommodation do i need to provide anything ? I don’t know if i should put the rental agreement of my villa (Started in June 2023) or book a refusable hotel through booking

About date of leaving what do you put inside ?

Last question, do I need to make a professional photography or can I use one made with my iPhone ? (I used my iPhone for my METV)

When the photo for the METV made with your iPhone was accepted there is no reason the iPhone cannot be uded for DTV. Better not use the same picture, because they might see it is not recent enough.

I used my phone for DTV application photo, first I got message it did not comply standards, made new photo and that was accepted, so it looks like check for correct photograph is automated.

Some embassies have very good websites about how to fill in the application, and how pictures should be.

There is also a manual in the top right corner of the e-visa website.

In my case, I am a foreigner in Malaysia resident. I have applied to obtain soft power medical treatment.

I provide hotel booking for a month in Bangkok nearby hospital. It went fine and no issue. Hope that helps you.
