Answer summary
The post inquires about the availability of a downloadable application form for the Certificate of Residence (CoR) in Thailand. Responses indicate that while no downloadable form is typically required, many immigration offices provide their own forms that must be filled out in person. Users suggest bringing your passport photos and other necessary documents such as a TM30 and lease agreement. It's noted that different offices may have varying requirements and forms. The CoR is issued for specific reasons, and separate certificates are provided for each purpose.

The TM 30 must be applied for by whomever you rent your "residence" from. Whether that is a hotel or an individual/company from which you rent a home. they can file on-line or in person at the immigration office.
I did this two weeks ago in Ubon Ratchathani. Ignore the form on the Thai Immigration website, it is out of date and each Immigration Office has their own version.
In addition to your TM30 make sure you also take your lease agreement or Yellow Book with you.
Be careful when selecting the reasons why you want a Residency Certificate as they will issue you with separate ones for each requirement.
For example, I wanted to open a bank account; buy a car; and apply for my Thai driving licence. I was given a separate Residency Certificate for each one. Note you cannot copy them, you must use the original you get from the Immigration Office.
It’s called TM18, they’ll give you this when you go there. Make sure you take your passport photos as they’ll need it to stick or the form and put a stamp on the top of the photo.
Last time I got one was in November, didn't have to fill anything out or provide a photo. The certificates were just Thai text which I didn't bother trying to read but they were accepted by the DLT
I'm just an occasional visitor so maybe, as they know you, you can just asked. My DL renewal was in January so after yours (also I was only on a 90 day Non-O so maybe that made a difference).
