Is the 30-day visa extension in Thailand added from the initial expiration date or the application date?

A friend from UK is here (Phuket) on 30 day visa exempt and wants to extend for another 30 days. Is the extension from the date of the expiration of the initial stamp or from the date of applying for the extension? Thanks
