How should I fill out my name on application forms if my passport does not have a separate middle name field?

I’m a bit confused about the middle name part. Normally, I do have a middle name, but on my passport, it only shows under “Name” and “Surname”, there’s no separate middle name field. So should I include my entire given name under “First Name”? But when I scanned my passport, it did place my middle name correctly, but I’m still not sure. As I said, even though I have two given names, they’re both listed under “Name” on the passport.

Put you name EXACTLY as it is on your passport. That is what the embassy check against. Any deviation from the name in your passport will result in rejection of your application

On my pasport bottom it's says only

van ballekom Josephus cornelis hendrik and there is no room for the last name pieter

Do I leave it out or add it ad my given names

On my US passport:

Surname = Last name

Given Names = First and Middle name (if you have a middle name)

Put your middle name where it belongs on the application.

Are you from a Spanish-speaking country? In my case, my passport says (I'm using other names as example):

Names: Juan Manuel

Family names: Pérez López

When I applied for the DTV I wrote

First name: Juan

Middle name: Manuel

Family name: Perez Lopez (without the tilde)

If you scan my passport in any machine, it pulls Perez Lopez Juan Manuel.

My application was approved.

It doesn’t say “names” in my passport, it says “name” but there is two into it. I think they will consider as a one name together, because I saw someone rejected he tought it should type into middle name instead of first name part but I don’t know what to do. I’m gonna mail the embassy but I just don’t wanna lose so much time until my return ticket to Thailand.

That's not my passport nor my country (couldn't find an image online), but mine's layout is exactly the same.

To be honest with the amount of rejections for names in here , I would advise email your embassy and ask them, it might take a few days for them to reply but it would be worth it
