How should I fill in my name on a visa application if my passport name format differs?

I am applying with Muay Thai soft power at Jakarta embassy. How should I fill in my English and Chinese names? Such as below:

Full name on passport: Anthony Bernard Chan Dak Eng

Machine readable field reads: Chan<<Anthony<Bernard<Dak<Eng

The system populates as:

First Name: Anthony

Middle Name: Bernard Dak Eng

Last Name: Chan

1) Is this correctly filled in?

2) If the visa comes out as "Anthony Bernard Dak Eng Chan", will I have any problems at immigration?

The confusion is because the Chinese name is "Chan Dak Eng".

First name: Anthony

Middle name: Bernard

Family name: Chan

Chinese name: Dak Eng

Dak Eng is populated into the middle name field. Is this correct?

It has to match your passport exactly so manual entry is nesseary if automatic population of the form is wrong. And it's wrong alot so good thing you checked and noticed this. It's #1 reason people get application rejected. They don't proof read it before sending

Dont trust the passport reader and type letter by letter one by one. If your name shows bla bla bla you write just that nothing else. Dont add take anything. When they ask you where you got your passport . PUT THE COUNTRY OF ISSUANCE . Never out consulate or any other thing EVEN IF ITS ON THE PASSPORT. Embassies and consulates that little piece of land are considered to be part of the same country. If your passport was issued at the us embassy in bangkok put place of issuance “ UNITED STATES”🙏

the embassy doesn’t have access to your scanned readable passport. They are taking note of your full name as it appears on the biodata page; i.e. surname; given names

Chan is the family name which is correct. First name is Anthony. Middle name is Bernard. Chinese name is Dak Eng which is entered into the Middle name field. Is this correct?

Anonymous participant if that is how it is formatted on your passport, then yes. It's difficult to confirm with 100% certainty without seeing the passport.

Family name is Chan which is correct. But I'm not sure if the first and middle name fields are populated correctly.

Anonymous participant how do first and middle names appear in your passport. If the names are not populated to mirror the passport, you must alter them. This is a common reason for rejection.
