You buy a re-entry permit at the immigration office that serves the area where you live (where you applied for your extension) OR at Suvarnabhumi Airport before you fly out
It is a half page stamp in your passport that is valid for the length of time your current stamp is good for. It lets you exit/re-enter the country and get stamped back in to that date.
A single re-entry permit is 1000baht at immigrations (1200 at Suvarnabhumi) and a multiple re-entry permit is 3800baht at immigrations (4000 at Suvarnabhumi)
then IF you are leaving you get a single re-entry permit and it will be valid only as long as your current stamp is <- meaning you get back IN to the country before that date and you'll be stamped in until then.
