The requirement is 90 days from (and including) day of entry. If you exit Thailand before then, no 90 day report is necessary.

Yes on your initial 90 day report

You can do the second online or via snail mail

What is initial

Initial is your initial 90 day report that you stay static in Thailand for 90 days

Initial is also anytime you return from an international trip

I’m not saying it happens regularly but some people do put reminders on calendar apps and do it
