yep you do. holding a yellow house book doesn't cut you any slack as far as filing a TM30 with the immigration office ;)

There is check out date in the form i dont know what should i add because my husband live at my house and no plan to move anywhere

You should be able to leave it blank. IF it doesn't allow you to leave it blank for whatever reason, then I'd use the late date of his entry stamp.
