When have DTV Visa and to stay here in thailand, do we need to exit other country after 180days and come again 180days till your five years of visa gone?

You can get an extension just prior to your first 180 days of each year however you must leave the country prior to the end of that extension.
So - Each year can do an extension and each year must leave once.

You have to exit minimum every 180 days, each entry gets you a six month stamp until the end of the 5 year validity period of the visa.
