Do I need the TM30 form for my visa extension application in Krabi?

My wife and I are on a 60 day TR visa with 21 days remaining. We will go to Krabi immigration in 6 days ( 15 days remaining on TR) to apply for 30 day extension. I found this TM30 online. Is this the correct form I need? Does my hotel have to fill this out? Do we need 1 form each or can we both go on this one. If they approve the extension does it start when my 60 day expires or the day I apply? We already have photocopies of passport and TM6 departure card as well as extra passport photos. Do I have all my ducks in a row? Thank you for any info. Cheers.

Krabi immigration does NOT ask for the TM30 , I have been there twice and never been asked for it.

Officers in Krabi are really friendly and helpful even when you arrive without any copies and your passport photo will you get the extension.

You need a copy of your arrival stamp , your passport, your departure card and of your tourist visa .

It's possible to book an appointment online but it was not busy at all two days ago.

If you have been staying at a regular hotels, it should have already been submitted by the hotel. Hotel just needs to provide you with proof.
