TLDR : Answer Summary
The discussion revolves around the process of renewing a car driving license in Thailand from a 2-year period to a 5-year period while holding a DTV visa. Most comments indicate that while initially, all holders receive a 2-year license, those with specific types of visas, especially Non-O visas, can qualify for a 5-year renewal. There is a consensus that additional documentation like a residency certificate and a medical exam are necessary for those seeking a 5-year license.
It’s possible , no problem according to DLT clerk Chiang Mai , just need residency certificate + medical exam paper. 505 Bath for each verhicle , car or motorcycle

I went in October to Rayong and got my first 2yr licence. There were two other guys there both on DTVs renewing their 2yr licence and were both given a 5yr!

As the DTV is classed as a tourist visa you would normally only get a 2 year licence

Everyone gets a 2 year license to start out with. Once the 2 years is up and you re-new you can get the 5 years license
Edit** I forgot to explain the need of a non-o visa to qualify for 5 year my bad
