Does anyone here actually know the internal process of the DTV application?

Once received by the embassy, does that particular embassy review all the documents and then chase everything up.

And then does that embassy then give final approval? Or does it get sent to Bangkok head office for approval?

TLDR : Answer Summary
The internal process of the DTV (Digital Nomad Visa) application typically involves local approval by the Thai embassy where the application is submitted. Most applications are approved at the local level, with exceptions primarily for specific nationalities such as Pakistani passport holders, which may require referral to the Bangkok head office. The embassy is responsible for reviewing all submitted documents and may request additional information from Bangkok if necessary.

The Evisa site is a Thai website … Embassies can bounce applications back and forth to Thailand if they think something needs sorting

It is approved locally on the vast majority of cases. The only ones heard of needing Bangkok approval so far is for Pakistan passport holders. I am sure it will be same for a few other passports too. That is exception rather than the rule.
