if he gets caught on overstay he will be fined, deported and blacklisted for 5 years. Why risk it ?

He should not wait. A real problem if they catch him before he goes to Immigration. Why take the risk? And YES. They can refuse to extend his visa. If so, they will give him 7 days (including that day) to leave Thailand.
