He is currently on overstay. If caught by Immigration or the police, he faces large fine, jail, deportation and ban from entering Thailand in the future. He needs to go to Immigration ASAP and pay his overstay fine of 500 thb per day and apply for an extension.

I try just one time: Do you know the official name of Immigration, it is: Royal Thai Police Immigration Bureau, this makes them all Police officers. if he gets caught on overstay he will be fined, deported and blacklisted for 5 years. Why risk it ?

He should not wait. A real problem if they catch him before he goes to Immigration. Why take the risk? And YES. They can refuse to extend his visa. If so, they will give him 7 days (including that day) to leave Thailand.
