Every immigration office has their own requirements. You should go to your immigration office and ask them for their list for the extension you are seeking. I believe guardian of a child is 400 or 500k thai baht in a bank account, among other things.

There are 2 guardian visas though, one is the father of a half thai child, and the other is the parent of a foreign child attending school in Thailand. The requirements are a bit different.

Yes you can get a 12 month extension if you meet the requirements which do not include insurance

If you meet the requirements, you can apply. For just a basic non-O, there is no health insurance requirement. You should go to your immigration office and ask them for their handout for a 1-year extension based on whatever type of non-O visa you have.
