okay, ONE more time. IF you had an O visa that started your 'chain of yearly extensions' you DO NOT need insurance. IF you had an O-A visa that started the 'chain of extensions' you DO need insurance.

, you can see by looking at the stamps in your passport that you originally came in on a 90 day Non-O visa entry the 15th of Nov 2016. That means you DO NOT need insurance.

it is for the Non Immigrant O-A visa and for those who has this visa and wish to stay longer using the Extension of Stay later. But as many people like to read and share all kind of doom scenarios, I guess something to do with Halloween party, it become a wild guess which one was fake or real.

No, you visited Sakon Nakhon on 30 Jan and got an Extension of Stay permitted up to 12 Feb 2020, as shown in picture 2. But you started the cycles of Extensions of Stay with the Non Immigrant O Visa as seen on picture 1, as you only got 90 days of stay. With this type of visa and Extensions of Stay there is no requirement for health insurance.
