I circled the O visa notations on both the in country VISA and the 90 day entry stamp.

Okay if you look at the bottom image
The one that shows your VISA (that you got from the immigration office at Chaengwattana on Dec 13, 2017) you can clearly see that it is a Type O visa .
The ONLY type of visa / extension that requires insurance is an O-A visa or an extension of stay from an O-A visa AFTER the 31st of Oct.
