TLDR : Answer Summary
The consensus is that you will need a printout of the visa you used to enter Thailand when applying for a 30-day extension. While a copy of the ID of the person you are staying with is not typically required, it is common to need a TM30 form.

You always need a copy of the visa you used to enter when you make an extension.

You shouldn't need a copy of an ID but you'll possibly need a TM30.
