To be sure, the nonimm O visa for retirement from the USA consulates require med insurance 40k outpatient and 400k inpatient, correct?

But this requirement does not carry over to an extension of stay, correct?

To avoid the need for insurance that the consulate wants (you’ll still need some for the ThailandPass) you can enter visa exempt or a regular tourist visa and then apply in country for a Non O visa and then extension from that. Needs a bit of fancy footwork for finances to be in place but is entirely doable.

correct, there is no requirement to carry the health insurance on Non-O visas when you get extensions inside the country.

That is a consulate specific thing when you buy them before comin' here
