Hi! We'll be going back to the US in a bit, and while there applying for our Non-O-over-50 visa for our return, with plans do to the retirement extension. For all that, we need to get an affidavit of our US marriage certificate notarized by the US consulate here in Chiang Mai. Gonna do that here before we leave for the US to just git 'er done. By chance, anyone have a blank online copy of the affidavit they use for that? I believe they have copies at the consulate, but was curious to have a look at it prior to our appointment at the consulate. Couldn't find one on the consulate site, just one to swear that you're not married.

We have affidavits from CM Consulate dated late December 2018. Our non o-a Visas are due for renewal at the end of May. Supposedly the Affidavits are valid for six month yet I’m hearing that they might not be in CM. We are thinking about moving to a different location anyway. Would the CM affidavits be acceptable in other provinces or would we have to fall back on the money in account scenario?

Income affidavits.

On May 29th 2019 Wife and I get to end of each of our two year stay on the USA Passport non o-a that we each have. Looking at postings I gather the following.

USA are still doing income affidavits until 31 December 2018 (corrected).

These affidavits are valid for 6 months with Thai IM for renewal. Correct?

Can the affidavits be issued at Chiang Mai consulate or does it have to be BKK Embassy?

My wife and I are USA passport holders. We are aiming for Phuket first and then Chiang Mai later this year for a holiday/vacation on a visa waiver on arrival. We want to obtain a 1yr Multi-Entry Retirement visa (O-A) while in CM. Below is the list of things I think we need for both to get the same visa.

5,000THB for the visa

3x Application form

CM USA Consulate - Cert of income and Police Check cert.

Are there any charges for these from the Consulate?

3x Photos

Medical Cert from local Dr.

Can all of this be obtained in CM or is a trip to Bangkok or outside Thailand be necessary?
