I am flying to BKK at the end of month with my DTV for the first time, it's my first international flight ever.
Can the lack of travel history cause any issues or prompt them to question me? I have the DTV based on remote job and have everything in order.

TLDR : Answer Summary
The user is concerned whether their lack of travel history might lead to complications upon arrival in Thailand with their Digital Nomad Visa (DTV). The community generally assures them that having a valid visa should prevent any significant issues. They recommend being prepared for potential standard questions at immigration, as it's the user's first international flight, but emphasize that this shouldn't cause major problems. Suggestions include having accommodation booked, carrying sufficient cash, and keeping visa documentation handy.
No problems, but they could potentially ask a few standard questions since it's your first DTV entry. Not a big deal. Just be nice and honest.

First, you have a visa so no worries there.
Second they have no way of knowing it's your first international flight.
Third neither would they care as it's the people who try to live year round in Thailand on tourist visas that they're bothered about - that's not you.
Fourth just to be sure you should have accommodation booked and carry

baht or equivalent cash in with you. Although you are extremely unlikely to be pulled up on this.
Fifth you should have a laminated copy of your DTV.
I don't think you need a flight out booked as a DTV holder.
I think that's everything. Let's see if anyone else has something to add or correct.

What are you afraid of if they ask you questions? You simply say that it's your first international flight and that you're looking forward to spending your vacation in a beautiful country like Thailand
