Should I have received a 90 day report document when I arrived in Thailand or upon my recent visa extension? I left the country in June with a re-entry stamp and recently had my visa extended until October 04. My date of re-entry was 19 June. So do I need to do 90 day report on 19 September as that will be 90 days? Like I said, I never received any document with a due date for the 90 day report

you never WILL receive any documents when you stamp in telling you when your 90 day report is due, YOU have to count it out on your own ๐
your next 90 day report is due 89 days after you stamped in to thailand, SO if you stamped in to the country on June 19th your 90 day report is due Sept 16th
you can apply in person from 2 weeks before down until a week after that date and you can apply online from 2 weeks before down until the due date
