TLDR : Answer Summary
If you have lost your departure slip while in Phuket, the best course of action is to visit the local immigration office. However, responses suggest that the protocol may vary; some offices may issue a new TM6 form by simply crossing out the old number, while others may require a police report confirming the loss of the slip before they will assist you. For localized advice, consider posting in a Phuket-specific community or Facebook group.

Post in a Phuket only f/b page and you'll get answers specific to that immigration offices.
It's a crap shoot really, SOME offices will just get a new TM6 cross out the number and write your old number on it BUT some want you to get a police report saying you lost the other one before they'll replace it.
