TLDR : Answer Summary
If your notification slip for the resident report was misplaced, ensure to file your 90-day report on time. Different immigration offices have varying policies regarding lost slips; some may allow you to proceed without a replacement, while others may require a police report stating the slip is lost. It might be helpful to bring someone who speaks Thai to assist you during the process.

Do as

said. But for me, I would take for free one of their staff with me. So he/she can explain in Thai language what happened and in case of Police report he/she can do the talking.

Just make sure you do your 90 day report on time. The actual policy of what you do varies by immigration office.

Some offices don't care if you lost the previous one and some offices want you to go get a police report stating it was lost before they'll let you do your next 90 day report.
