What should a bank letter include for a yearly extension of stay in Thailand?

As next year I will be using the 'Banked amount' (as UK embassy is no longer issuing letters) when applying for my yearly extension of stay, I just have one question, can anyone tell me what the bank letter should say?, should it be addressed to a certain individual (I apply in Samut Prakan). Does it need my passport number?, or other identifying data ?. Is there a 'form letter' you can get from Immigration?. Thanks.
