What is the third document that a Thai spouse needs to fill out for a Non-O Marriage extension application?

Good morning. I applied for my yearly Non-O Marriage extension last week and noticed there were three new forms that we had to fill out at the counter: ST.11 and STM.10. I canโ€™t remember the third one that my wife had to fill out in Thai. They are not listed on the Chaengwattana list of documents. Does anyone know the name of that third document that the Thai spouse fills out to validate the marriage? Also, I need to get the stamp next week and apply for the re-entry stamp. Are there any new documents required for this now too beside the TM. 8?

I recently did same in Nong Bua lamphu and was told it is now a requirement to have a marriage certificate validated at the local government office on the day you apply the same as bank statements. This is new for me but not a great hassle.

nope just show up, get the year extension inked in and then go buy a re-entry permit with a filled out TM8 (application for re-entry) copies of the data page of your passport, your most recent entry stamp to thailand and the new yearly extension you just got inked in along with a passport sized photo

STM.10 (Acknowledgement of Family Relationship to Thai National) <- for marriage raising thai child extensions

STM.11 (Consent Form for Fact-Checking and Information verification)
